Responsibilities Assist residents or clients with activities of daily living (ADLs), including personal hygiene, dressing, grooming, and mobility support Monitor patient health by observing vital signs, managing medication administration, and reporting changes to healthcare teams Support residents with meal preparation, feeding, and ensuring nutritional needs are met according to care plans Utilize patient transfer methods such as Hoyer lifts and manual handling techniques to ensure safe movement and transfers Maintain accurate documentation of care provided, patient observations, and adherence to HIPAA regulations for confidentiality Provide companionship and emotional support to residents with dementia, Alzheimer's disease, or other neurodevelopmental disorders Assist with home & community care tasks such as transportation assistance, light cleaning, and maintaining a safe environment Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly preferred Demonstrated knowledge of dementia care, memory care, hospice care, and Alzheimer's support strategies Hands-on experience with patient monitoring tools and techniques including vital signs measurement and medication administration Familiarity with care plans development and implementation tailored to individual needs Experience working with individuals with developmental disabilities or neurodevelopmental disorders such as autism or intellectual disabilities support Certification in CPR and first aid is required; additional certifications in manual handling or heavy lifting are advantageous Strong communication skills for effective client interaction and team collaboration in diverse caregiving environments Join us in delivering exceptional care that transforms lives!
