Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene care. Support clients with mobility and transfers using appropriate manual handling techniques and equipment such as Hoyer lifts and patient transfer methods. Monitor vital signs, observe patient conditions, and report any changes or concerns promptly to healthcare teams. Prepare nutritious meals tailored to dietary needs and assist with meal planning and preparation. Administer medications following care plans and ensure compliance with HIPAA regulations to maintain confidentiality. Provide transportation assistance for appointments, errands, or community activities to promote independence. Maintain a clean, safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces. Document care activities accurately in client records, including patient monitoring data and progress notes. Support clients with specialized needs such as dementia care, Alzheimer's care, autism support, or neurodevelopmental disabilities through personalized approaches. Uphold residents' rights by respecting privacy, promoting autonomy, and fostering a positive atmosphere aligned with long-term care standards. Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to observe patients carefully and respond appropriately to their needs. Knowledge of care plans, medication administration, patient transfer methods, and vital signs measurement. Certification in CPR and First Aid is required; additional certifications such as HIPAA training are advantageous. Experience working with individuals with developmental disabilities, intellectual disabilities, or neurodevelopmental disorders is a plus. Strong communication skills for effective client interaction and team collaboration. Ability to lift heavy objects safely and perform manual handling tasks while maintaining proper body mechanics.
