Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity. Support patients with mobility and transfers using appropriate techniques such as Hoyer lifts and patient transfer methods to promote safety. Monitor vital signs and observe patient conditions regularly to detect any changes or concerns promptly. Prepare nutritious meals aligned with dietary needs and assist with meal planning and cooking to promote health and well-being. Administer medication following care plans while maintaining strict adherence to HIPAA regulations and confidentiality standards. Provide companionship and emotional support, fostering positive communication and engagement tailored to individual needs. Assist with light housekeeping tasks such as cleaning, laundry, and maintaining a safe living environment to support overall health. Support clients in community outings or transportation needs while ensuring their safety during travel or appointments. Qualifications Proven experience working in home health, assisted living facilities, nursing homes, or group homes is highly preferred. Strong knowledge of patient observation, monitoring vital signs, medication administration, and dementia or Alzheimer's care. Experience working with individuals with developmental disabilities, neurodevelopmental disorders, or autism is a plus. Ability to perform manual handling tasks including heavy lifting and patient transfers safely and effectively. Certification in CPR and First Aid is required; additional certifications in hospice care or geriatrics are advantageous. Familiarity with patient rights, HIPAA compliance, care plans, and home & community care standards. Excellent communication skills for client interaction and team collaboration; compassionate approach essential for success.
