Responsibilities Provide personal care assistance including bathing, grooming, dressing, and mobility support to clients with various disabilities or age-related needs Assist with Activities of Daily Living (ADLs) such as toileting, feeding, and medication reminders in accordance with care plans Monitor patient health by checking vital signs, observing symptoms, and reporting changes promptly to healthcare professionals Support clients with meal preparation and nutrition needs, ensuring dietary restrictions are followed accurately Help with patient transfer methods using equipment such as Hoyer lifts or manual handling techniques to ensure safety during mobility tasks Maintain a clean and safe environment by performing light housekeeping duties including cleaning and laundry tasks Facilitate transportation for medical appointments or community outings while ensuring client safety and comfort Skills Proven experience working in home health, assisted living facilities, nursing homes, or group homes is highly preferred Strong knowledge of patient observation techniques, medication administration, and vital signs monitoring Familiarity with dementia care, Alzheimer's care, memory care, and hospice support practices Ability to assist with complex patient needs such as using Hoyer lifts or patient transfer methods safely Excellent communication skills for effective client interaction and documentation of care activities Knowledge of HIPAA regulations to ensure confidentiality and privacy of patient information Experience working with individuals with autism, developmental disabilities, neurodevelopmental disorders or intellectual disabilities support is advantageous Capable of performing heavy lifting when necessary while adhering to manual handling safety standards CPR and First Aid certification are required; ongoing training is supported for continued professional development Join us in delivering compassionate home care that empowers individuals to thrive in their own environments.
