This vital role offers an opportunity to make a meaningful difference in the lives of clients by delivering personalized care that promotes independence, safety, and well-being. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene Monitor patients' vital signs and observe changes in health status, reporting any concerns promptly Prepare nutritious meals aligned with dietary restrictions and preferences, ensuring proper nutrition Support medication administration following care plans and HIPAA regulations to ensure privacy and safety Facilitate patient transfer methods using manual handling techniques and equipment such as Hoyer lifts or patient transfer devices Maintain detailed documentation of patient care activities, observations, and progress in compliance with healthcare standards Provide companionship and emotional support to enhance clients' mental well-being and reduce feelings of isolation Skills Proven experience working with individuals with developmental disabilities, neurodevelopmental disorders, or aging populations such as geriatrics or dementia patients Knowledge of Alzheimer's care, memory care, hospice care, and managing residents with dementia or other cognitive impairments Familiarity with home health procedures including patient monitoring, medication administration, first aid, CPR, and vital signs assessment Experience working in group homes, assisted living facilities, nursing homes, or long-term care environments Ability to perform heavy lifting safely and assist with manual handling tasks using appropriate patient transfer methods Strong communication skills for effectively engaging with clients and collaborating with healthcare teams while respecting HIPAA guidelines Competence in meal preparation, cleaning duties related to caregiving environments, and providing transportation assistance when needed Join us in delivering compassionate care that transforms lives!
