Responsibilities:
Provide direct, hands-on personal care and daily living assistance to clients in home and community settings Support clients with activities of daily living (ADLs) including bathing, grooming, dressing, and meal preparation Accompany and assist clients with community outings, appointments, and social activities Monitor and document client health, behavior, and progress, reporting changes to supervisors Implement individualized care plans tailored to each client's unique needs and goals Foster a safe, supportive, and dignified environment for all clients Build trusting relationships with clients, families, and care team members

8:00am-2:00pm M-F Family prefers female caregiver and dog friendly person Prior experience in home care, personal care assistance (PCA), direct support, or a related caregiving role preferred Genuine compassion, patience, and dedication to supporting individuals with diverse needs Ability to pass a background check as required by Minnesota state regulations Valid driver's license and reliable transportation preferred Strong communication and interpersonal skills Ability to work independently and as part of a collaborative care team
