Position Summary Homeward Bound is seeking an Individual Engagement DSP to provide dedicated 1:1 support to an individual we serve.

Hours:

Monday thru Friday 9:00am-5:00pm This position is centered on building a meaningful connection with the individual, learning their interests and preferences, and creating opportunities for purposeful engagement throughout their day. Engagement is based on what is meaningful to the individual and may take place within the home, in the community, or a combination of both. Rather than focusing simply on completing activities or outings, the Individual Engagement DSP helps the individual stay engaged, connected, and involved in ways that reflect their interests, choices, abilities, and personal goals. Responsibilities Provide consistent, individualized 1:1 support based on the person's needs, interests, preferences, and goals. Build a positive and supportive relationship with the individual through meaningful interaction and engagement. Identify and facilitate activities that are enjoyable, purposeful, and appropriate for the individual. Support engagement within the individual's home, such as hobbies, games, music, creative activities, cooking, exercise, conversation, or other preferred activities. Support community participation and outings when desired by the individual, including recreational, social, cultural, or other activities of interest. Encourage choice, independence, self-expression, and participation in daily life. Provide assistance with activities of daily living and personal care as needed. Monitor the individual's health, safety, and well-being and respond appropriately to their needs. Complete medication administration and implement support and care plans as assigned. Collaborate with the individual's support team to understand goals, preferences, and successful engagement strategies. Document services, activities, progress, and relevant observations accurately. Maintain a safe, respectful, and supportive environment. Protect confidentiality and follow HIPAA requirements and Homeward Bound policies. Qualifications At least 1 year of experience supporting individuals with disabilities is required. Must have valid MN Driver's License and acceptable driving record. Must pass a Minnesota Department of Human Services Background Study. Genuine interest in building relationships and helping individuals participate in activities that are meaningful to them. Patient, dependable, creative, and comfortable adapting activities based on an individual's interests and needs. Ability to communicate effectively in English, both verbally and in writing. Ability to accurately count and handle money. Ability to assist with activities of daily living (ADLs), including bathing, toileting, dressing, and meal preparation. Ability to support individuals of any gender with personal care needs. Ability to follow health monitoring, medication, and individualized support procedures. Ability to lift and transfer up to 50 lbs. Commitment to respecting each individual's dignity, choices, preferences, and independence.
