of Isanti. We are looking for a Hospice Care Partners to work 32 hours per week, including every other weekend and every other Holiday. We take pride in providing compassionate, exceptional care and comforting moments for our patients. Hospice care partners play a vital role in the care that we provide. You should be able to follow instructions and perform a variety of tasks to help our patients. Hospice care partners must have a valid driver's license and reliable transportation in order to provide care wherever our patient's call home. Availability:

Full-Time Days, Alternating Weekends/Holidays Responsibilities:

Provide patients with assistance for activities of daily living (walking, bathing, bathroom needs, etc.) Perform light housekeeping duties Be a comforting and supportive companion for hospice patients and their loved ones Follow individualized care plans for patients Coordinate with all members of the hospice team Report changes to patient condition to the hospice nurse Report any unusual incidents Adhere to organization's documentation and reporting standards Act quickly and responsibly in cases of emergency

Skills/Qualifications:

Must be 18 years of age or older Knowledge of housekeeping activities Willingness to adhere to health and safety standards Respectful and compassionate Good time management skills Ability to work independently Outstanding communication and interpersonal skills Strong ethics Physical endurance Valid driver's license, transportation, and auto insurance Minimum of 1 year of caregiving experience (Preferred) CNA certification (Preferred) High school diploma or
