Independent Living Partners is seeking experienced, driven, and professional ( Spanish-English bilingual ) IHS worker to join our team to provide Individualized Home Supports, homemaking, and other similar services in the Rosemount area. We are looking for highly responsible, kind, patient, and professional caregivers. Experience in IHS/ILS, PCA, housekeeping tasks, and caring for the elderly or individuals with physical and behavioral challenges will make you a strong candidate. Individualized Home Supports (IHS) is an active role that involves bending, walking, and hands-on work. You will not only assist clients with various activities but also work alongside them to ensure tasks are completed efficiently. In many cases, you will independently complete activities and tasks that help individuals maintain a dignified and independent life. Responsibilities Personal Care & Daily Living Support Provide bowel and bladder care. Assist the individual in receiving their medication on time. Support the individual with limited mobility. Assist with eating, dressing, bathing, mobility, transfers, toileting, positioning, and personal hygiene. Assist with meal preparation according to the individual's preferences and health restrictions. Perform necessary housekeeping tasks to maintain a clean environment. Provide emotional support and companionship throughout the care process. Act quickly and responsibly in emergencies. Communicate with the individual's care team regarding their care. Advocate for the individual's well-being. Maintain professional relationships with the client's family members or friends as required by the role. Adhere to health and safety standards and goals. Provide respite for caregivers. Appointment Coordination & Transportation Review the client's weekly appointment schedule at the start of each shift and confirm times, locations, and any preparation needed. Provide safe, on-time transportation to and from medical, therapy, and other scheduled appointments, allowing sufficient time for the client to board and exit the vehicle. Accompany the individual into appointments as requested, remaining present to support communication, mobility, and comfort. Assist with completing intake paperwork, insurance/ID information, and appointment-related forms. Communicate schedule changes, delays, or missed appointments promptly to the client's care team and supervisor. Assist in setting up and confirming future appointments and coordinating around the client's routine. Maintain accurate documentation of appointments attended, including case notes and timecards, in accordance with EVV and agency policy. Community Access & Independence (as applicable) Assist with arranging for or providing transportation for community activities beyond appointments. Assist the individual in accessing the community safely for socialization with friends, family, and peers. Assist with shopping for groceries and other essentials, including household pet supplies as needed. Assist with paperwork, organization, and daily case noting/timesheets. If the client has pets in the home, assist with basic pet care (e.g., feeding, water) only as directed by the client and outlined in the care plan. Qualifications Be at least eighteen (18) years of age. Applicants between sixteen (16) and seventeen (17) years of age may be considered, provided the worker's parent or guardian gives written permission to work with a client. Must have successfully completed the mandatory PCA Standardized Training and passed the exam with a score of 80% or higher, and must provide a legible certificate or the email used to look up the certificate in the Minnesota DHS training system. Must demonstrate to the qualified professional the ability to provide personal care services by accurately following the client's care plan. Must be able to work with minimal direct supervision, exercise sound judgment, and know how and when to report changes in the client's condition to the qualified professional. Must demonstrate dependability, tact, punctuality, and the ability to follow instructions — reliability is essential given the client's fixed weekly appointment schedule. Must be in good physical and mental health. Must be legally authorized to work in the United States. Must pass a criminal background check (Minnesota DHS Background Study). Must have reliable, insured transportation and a working communication device. Must hold a valid driver's license with fewer than 3 moving violations in the last 3 years (including DUIs). No known severe allergies to household pets (dogs, cats, or other animals that may be present in the client's home) that cannot be safely managed; must disclose any pet-related allergies or sensitivities during onboarding.
