  • Full-time Position Summary The Home Health Aide (HHA) provides compassionate, person-centered care to residents in our assisted living community.
You will assist residents with Activities of Daily Living (ADLs), promote independence, maintain a safe and clean environment, and support the overall well-being of residents while following individualized service plans. This role requires reliability, respect for resident dignity and privacy, and strong teamwork with nursing and other staff.
Key Responsibilities Personal Care & ADLs :
Assist residents with bathing, showering, grooming, oral hygiene, dressing, toileting, incontinence care, skin care, and transfers/mobility using proper techniques and assistive devices.
Daily Living Support :
Help with meal preparation, serving, and feeding; encourage resident participation in dining and activities; provide companionship and emotional support.
Housekeeping & Environment :
Perform light housekeeping tasks such as laundry, changing bed linens, tidying resident rooms, and maintaining a clean, safe living space.
Health Monitoring & Reporting :
Observe residents for changes in physical, mental, or emotional condition; accurately document cares, vital signs (as trained), intake/output, and report concerns promptly to nursing staff or supervisor.
Medication Assistance :
Assist with self-administration of medications as permitted by state regulations and facility policy (under RN or qualified staff supervision).
Activity Participation :
Encourage and assist residents in life-enriching activities, social engagement, and recreational programs as outlined in their Individualized Service Plan (ISP).
Safety & Compliance :
Follow infection control procedures, emergency protocols, resident rights, and all facility policies. Maintain HIPAA confidentiality and report any incidents or elopement risks immediately.
Team Collaboration :
Work closely with the Health Services team, participate in care plan meetings, and communicate effectively with families and staff. Qualifications High School diploma or GED preferred. Current Home Health Aide (HHA) certification or equivalent (e.g., PCA certification in Minnesota) required or ability to obtain within 30-60 days. Previous experience in assisted living, long-term care, home health, or caregiving preferred. Current CPR/BLS and First Aid certification preferred. Ability to read, write, and speak English fluently; basic computer skills for documentation. Must pass background check, TB test, and drug screen (per MN regulations). Physical ability to lift up to 50 lbs, stand/walk for extended periods, and assist with transfers. Preferred Skills & Attributes Compassionate, patient, and resident-focused attitude. Strong observation, communication, and problem-solving skills. Reliability with flexible scheduling, including evenings, weekends, and holidays as needed. Knowledge of Minnesota assisted living regulations, resident rights, and infection control practices. What We Offer Competitive hourly wage based on experience.
