Job Description
LPN -Full-Time/Days Opportunity in Assisted Living at Woodcrest of Country Manor, St. Joseph, MN As an LPN at Woodcrest you will join an award-winning Assisted Living team, ensuring exceptional care for all residents. In this role the LPN will work alongside our established, committed nursing team under the guidance of the RN Case Manager and Executive Director of Clinical Assisted Living. Responsibilities for
LPN:
Assist in the development of resident care plans, and participate in the review of care plans as requested Provide nursing services in accordance with the established care plan Make routine resident visits to assess and document physical and emotional status; implement any required nursing interventions Assist residents to achieve and maintain the highest level of self-care and independence Administer and maintain records of prescribed medications and treatments Communicate pertinent information to supervising RN, physicians, family, and/or other appropriate personnel Accurately complete admission, weekly, follow-up, and discharge charting as requested Provide support, assistance, direction, and supervision to other nursing and support staff as needed Maintain positive and effective communication and relationships with the patients and their families Other duties as assigned
Qualifications:
Current license in good standing as an LPN in the State of Minnesota CPR certification Previous experience in nursing or other medical fields Ability to act as a professional representative for Woodcrest's Assisted Living Department Familiarity with medical tools and equipment Ability to build rapport with residents Compassionate and caring demeanor Excellent written and verbal communication skills Woodcrest (established in 2017) is a beautiful, senior living community that offers a unique environment.
