Duties Provide comprehensive personal care assistance, including help with bathing, dressing, grooming, and mobility support using proper patient transfer methods and manual handling techniques. Monitor patients' vital signs and observe for any changes in health status, documenting findings accurately while adhering to HIPAA regulations to ensure confidentiality. Support clients with activities of daily living (ADLs) such as meal preparation, light housekeeping, cleaning, and transportation to appointments or community activities. Implement specialized care for residents with dementia, Alzheimer's, or neurodevelopmental disorders by applying memory care techniques and engaging in therapeutic activities. Use equipment like Hoyer lifts safely during patient transfers and assist with hospice or palliative care needs compassionately. Communicate effectively with clients and their families about care progress, concerns, and updates to foster trust and transparency. Transportation to and from doctor appointments or other errands required for the clients. Skills Proven experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care environments such as assisted living or nursing homes. Strong patient observation skills to detect subtle changes in health status or behavior patterns. Knowledge of home health practices including patient monitoring, medication administration, and patient personal care assistance. Ability to perform CPR and first aid confidently; familiarity with HIPAA compliance is essential. Competence in caring for residents with dementia or Alzheimer's disease through specialized memory care strategies. Experience operating patient transfer equipment like Hoyer lifts and performing manual handling safely. Excellent communication skills to coordinate effectively with clients, families, healthcare teams, and social workers.
