Job Summary We are seeking a compassionate and dedicated Personal Care Aide to join our team, providing essential support to individuals in various care settings. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' vital signs and observe for any changes in health status or behavior Support medication administration and ensure adherence to care plans in compliance with HIPAA regulations Prepare nutritious meals tailored to individual dietary needs and assist with feeding as necessary Use proper patient transfer methods and manual handling techniques, including Hoyer lifts and patient transfer devices Maintain a clean, safe environment through regular cleaning and sanitation of client spaces Provide personal transportation assistance for appointments or community outings Document patient observations, care activities, and communication with healthcare teams accurately and promptly Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred Knowledge of dementia care, Alzheimer's care, memory care, and neurodevelopmental disorders such as autism or intellectual disabilities support Ability to perform patient monitoring, vital signs measurement, and basic first aid including CPR certification Familiarity with care plans, medication administration procedures, and HIPAA compliance standards Experience working with individuals with disabilities or developmental delays is advantageous Strong communication skills for effective client interaction and teamwork Physical ability to lift heavy objects safely and assist with patient transfers using appropriate techniques Valid certifications in first aid and CPR are required; additional certifications related to hospice or geriatrics are a plus Join us to provide compassionate care that truly makes a difference!
