Home Care Coordinator Help Build Something Special in the Triad Join the Founding Team at Golden Harmony Home Care Golden Harmony Home Care is launching a brand-new office in North Carolina's Triad, and we're looking for a Home Care Coordinator who is excited to help build something exceptional.

This isn't just another scheduling position. As we expand into the Triad, we're looking for team members who want to help build something lasting—not simply fill a position. Position Summary The Home Care Coordinator is one of the most important roles in our organization.

This position combines care coordination, scheduling, client service, caregiver support, recruiting assistance, and office operations into one dynamic role. Every day brings new challenges, changing priorities, and opportunities to positively impact the lives of our clients and care professionals.

You'll work closely with the Area Director to coordinate care, support caregivers, solve scheduling challenges, maintain regulatory compliance, and help grow the new office through exceptional customer service. This is a full-time, onsite position. What You'll Do Client Care & Relationship Management Build trusted relationships with clients, families, caregivers, and referral partners through proactive communication and exceptional service. Meet with prospective clients and families to understand their needs and assist with in-home consultations. Assist in developing, updating, and maintaining individualized care plans. Conduct client visits and follow-up calls to ensure satisfaction and address concerns before they become problems. Help maintain Golden Harmony's exceptional client satisfaction and Net Promoter Score (NPS). Scheduling & Care Coordination Coordinate caregiver schedules to ensure every client receives consistent, high-quality care. Fill open shifts quickly while making thoughtful caregiver-client matches. Respond to caregiver call-outs and last-minute schedule changes with urgency and professionalism. Communicate schedule changes proactively with clients, families, and caregivers. Monitor schedules throughout the week to ensure billing and payroll accuracy. Create and maintain scheduling calendars within our scheduling software. Collaborate with our scheduling team to maximize consistency, continuity of care, and billable hours. Caregiver Experience Partner with our Recruiting Team to interview, select, and onboard exceptional caregivers. Support orientation and onboarding while helping establish the culture and expectations of our new office. Build positive relationships with caregivers through regular communication, coaching, encouragement, and accountability. Reinforce Golden Harmony's Core Values and service standards in every interaction. Help caregivers feel supported, appreciated, and successful. Office Operations & Compliance Maintain accurate client and caregiver records, including care plans, schedules, notes, and documentation. Ensure compliance with North Carolina Department of Health and Human Services regulations and company policies. Assist with audits and documentation reviews as needed. Participate in the shared after-hours on-call rotation. Maintain communication with leadership regarding challenges, opportunities, and operational improvements. Perform additional responsibilities that support the success of the team and our clients. What We're Looking ForPreferred Experience Two or more years of experience in home care, senior living, healthcare staffing, scheduling, care coordination, recruiting, or a similar customer-service environment. Experience coordinating schedules involving multiple clients and employees. Comfortable using scheduling software, CRM systems, Microsoft Office, and Google Workspace. Requirements Valid driver's license and reliable transportation. Ability to travel throughout the Triad service area for client visits and meetings. Ability to participate in an after-hours on-call rotation. Ability to sit, stand, walk, and occasionally lift up to 25 pounds. Ability to work full-time onsite during regular business hours.
