Job Description
Responsibilities Deliver personalized patient care including assistance with activities of daily living Monitor patients' vital signs, observe changes in condition, and accurately document health status in care plans while respecting residents' rights and confidentiality under HIPAA regulations. Administer medications safely following prescribed instructions, ensuring proper medication administration techniques are followed. Support patients with meal preparation, nutritional needs, and assisted living routines to promote health and well-being. Assist with patient transportation within the community or home environment, providing personal transportation assistance as needed. Maintain a clean and safe environment by performing light cleaning duties and adhering to infection control protocols. Communicate effectively with clients, families, and healthcare teams to ensure coordinated care and address individual needs related to dementia care, Alzheimer's care, autism spectrum disorders, or other neurodevelopmental disabilities. Requirements Proven experience working with individuals with disabilities, developmental disabilities, or senior care in settings such as group homes, nursing homes, assisted living facilities, or home health environments. Knowledge of patient observation techniques, dementia care strategies including memory care practices, and familiarity with resident's rights legislation. Certification in CPR and First Aid is required; additional certifications such as manual handling training or HIPAA compliance are preferred. Ability to perform heavy lifting safely and use patient transfer methods like Hoyer lifts; experience working with patients requiring mobility assistance is highly valued. Strong communication skills for effective client interaction and team collaboration; experience working with individuals with autism or neurodevelopmental disorders is a plus. Flexibility to adapt to various client needs across different settings including hospice care or long-term care environments. Valid driver's license for personal transportation assistance when necessary; experience supporting geriatrics or home & community care is advantageous.
