Responsibilities Deliver personalized patient care including assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods like Hoyer lifts or manual handling techniques. Monitor patients' vital signs and observe for any changes in health status, documenting findings accurately in care plans while ensuring compliance with HIPAA regulations. Administer medications safely following prescribed instructions and assist with medication management to ensure proper adherence. Prepare nutritious meals tailored to individual dietary needs and assist clients with meal consumption as needed. Support clients with cognitive impairments by implementing memory care strategies, Alzheimer's or dementia-specific routines, and supporting residents' rights within assisted living or long-term care environments. Assist with transportation for medical appointments, community outings, or personal errands to promote independence and social engagement. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces. Engage in patient observation to detect signs of distress or health deterioration promptly while communicating effectively with healthcare teams or family members. Qualifications Proven experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly desirable. Strong background in patient monitoring, medication administration, CPR/First Aid certification, and manual handling techniques including heavy lifting and patient transfer methods. Familiarity with dementia care, Alzheimer's support, intellectual disabilities support, neurodevelopmental disorders, autism spectrum disorder (ASD), and developmental disabilities is preferred. Knowledge of HIPAA regulations and experience maintaining confidentiality of patient information is essential. Ability to perform light housekeeping tasks such as cleaning and meal preparation while supporting clients' personal hygiene needs. Excellent communication skills for client interaction and collaboration with healthcare professionals or family members.
