Job Overview Relief Home Care Agency is seeking a dedicated and compassionate PRN-Overnight Personal Care Aide/ In-Home Aide or CNA to join our team in providing exceptional care to individuals in a variety of residential and community settings. As a Personal Care Aide, you will play a vital part in supporting residents' daily needs, ensuring their safety, comfort, and dignity through personalized care and attentive monitoring. Duties Assist with personal care and activities of daily living Provide toileting and incontinence care as needed Assist with safe mobility and transfers as permitted by the care plan Provide companionship and supervision Maintain a clean and safe client environment Observe and report changes in the client's condition Follow the client's plan of care and agency policies Prepare nutritious meals aligned with dietary plans, and assist with feeding as needed to support optimal health Utilize patient transfer methods such as Hoyer lifts or manual handling techniques for safe mobility assistance Maintain cleanliness of resident living areas through light cleaning duties, ensuring a hygienic environment Document care provided, update care plans regularly, and communicate effectively with team members about residents' needs and progress Support residents with neurodevelopmental disorders or dementia by implementing specialized memory care strategies and Alzheimer's/dementia care techniques Follow all safety procedures including first aid, CPR certification requirements, and proper use of patient transfer equipment Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable Knowledge of patient observation techniques and monitoring vital signs is essential Experience with Alzheimer's care, memory care, hospice care, or working with individuals with disabilities enhances your suitability for this role Ability to perform heavy lifting safely using proper patient transfer methods and manual handling skills Strong communication skills for effective client interaction and team collaboration Certification in CPR and First Aid is required or must be obtained upon hire Familiarity with HIPAA regulations to ensure confidentiality of patient information Experience working with individuals with autism or neurodevelopmental disorders is a plus Ability to work overnight shifts reliably while maintaining high standards of patient personal care assistance Join us in making a positive impact on the lives of those we serve!
