Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene. Support patients with mobility and transfers using proper techniques such as Hoyer lifts and patient transfer methods to ensure safety. Monitor vital signs, observe patient conditions, and report any changes to healthcare professionals promptly. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation. Administer medications accurately following care plans while maintaining compliance with HIPAA regulations to protect patient privacy. Provide companionship and emotional support, fostering a positive environment that encourages social interaction and mental stimulation. Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe living space to promote overall health and hygiene. Support clients with transportation for appointments or errands, ensuring safe and reliable personal transportation assistance. Document care activities clearly in accordance with agency protocols and communicate effectively with clients, families, and healthcare teams. Assist with specialized care needs including Alzheimer's care, dementia support, memory care, hospice care, or working with individuals with autism or developmental disabilities. Experience Prior experience working in home health, assisted living facilities, nursing homes, or group homes is highly preferred. Demonstrated ability to provide patient personal care assistance with compassion and professionalism. Knowledge of medication administration, patient monitoring techniques, first aid, CPR certification, and manual handling skills. Familiarity with care plans tailored for geriatrics, neurodevelopmental disorders, intellectual disabilities support, or long-term care environments. Experience working with individuals with disabilities or neurodevelopmental disorders enhances your ability to deliver specialized support. Strong communication skills for effective client interaction while respecting residents' rights and privacy standards under HIPAA regulations. Ability to perform heavy lifting safely when necessary and maintain a high level of attention during patient observation tasks.
