Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene. Support patients with mobility and transfer methods using equipment such as Hoyer lifts and patient transfer techniques. Monitor vital signs and observe patient conditions to identify any changes or concerns, ensuring timely communication with healthcare providers. Prepare nutritious meals aligned with dietary restrictions and preferences, ensuring proper food safety practices. Administer medications accurately following care plans and HIPAA regulations to maintain confidentiality and safety. Provide companionship and emotional support to promote mental well-being, especially for clients with dementia, Alzheimer's, or neurodevelopmental disorders. Assist with light housekeeping tasks such as cleaning, laundry, and maintaining a safe living environment. Support transportation needs for medical appointments or community activities, ensuring safe and reliable client mobility. Follow established care plans diligently while documenting observations, interventions, and client progress accurately. Maintain compliance with all safety standards including first aid, CPR certification, manual handling procedures, and HIPAA privacy regulations. Qualifications Proven experience working as a caregiver in home health, assisted living, nursing home settings or group homes. Knowledge of patient observation techniques and ability to monitor vital signs effectively. Experience providing dementia care, memory care, Alzheimer's support or hospice care is highly desirable. Familiarity with medical equipment such as Hoyer lifts and patient transfer methods; CPR and first aid certification required or willing to obtain upon hire. Strong communication skills to effectively interact with clients, families, and healthcare teams. Ability to perform heavy lifting safely when assisting with patient transfers or mobility needs. Experience working with individuals with disabilities or neurodevelopmental disorders is a plus. Knowledge of medication administration protocols and care plan implementation. Ability to handle manual handling tasks safely while maintaining client dignity and comfort. Valid driver's license and reliable transportation preferred for client outings or appointments.
