In this role, you will provide comprehensive assistance to clients in their homes, promoting independence, safety, and well-being. Responsibilities Deliver personalized patient care, including assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs, managing medication administration, and updating care plans accordingly Assist with meal preparation and nutritional needs to promote overall health and well-being Support patients with memory care needs, including Alzheimer's and dementia care, ensuring safety and comfort at all times Facilitate patient transfers using appropriate methods such as Hoyer lifts or other manual handling techniques while ensuring safety standards are maintained Provide transportation assistance for appointments or errands, ensuring safe and reliable client mobility within the community Maintain detailed documentation of patient observations, care activities, and communication with healthcare teams in compliance with HIPAA regulations Perform light cleaning duties to uphold a safe and hygienic environment for clients Engage in active communication with clients and their families to ensure care needs are met effectively and compassionately Experience Prior experience working in home health, assisted living facilities, nursing homes, or group homes is highly preferred Demonstrated knowledge of patient observation techniques, vital signs monitoring, medication management, and first aid/CPR certification Familiarity with dementia and Alzheimer's care practices, including memory care strategies and behavior management Hands-on experience assisting with complex transfers using assistive devices like Hoyer lifts or patient transfer methods Background in geriatrics or long-term care environments that emphasizes compassionate caregiving and client-centered support Strong communication skills for effective client interaction and collaboration with healthcare professionals Join us in providing exceptional home & community care that empowers individuals to live their best lives.
