Responsibilities Deliver personalized patient care, including assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support Monitor patients' health by observing vital signs, managing medication reminders, and reporting changes to healthcare professionals Assist with meal preparation and nutritional needs tailored to individual dietary requirements Support patients with mobility using equipment like Hoyer lifts and proper transfer techniques to ensure safety during movement Maintain detailed care plans, document patient progress, and adhere to HIPAA regulations to protect client confidentiality Provide companionship and emotional support, fostering a trusting relationship that promotes mental well-being Assist with transportation for medical appointments or community activities, ensuring safe and reliable travel Experience Prior experience working in home health, assisted living facilities, nursing homes, or group homes is highly preferred Demonstrated expertise in patient observation, dementia care (including Alzheimer's and memory care), and neurodevelopmental disorders support Knowledge of HIPAA standards, medication administration protocols, and patient monitoring techniques Hands-on experience with manual handling equipment such as Hoyer lifts and patient transfer methods Familiarity with caring for individuals with developmental disabilities or intellectual disabilities support needs Strong communication skills for effective client interaction and collaboration with healthcare teams Embark on a fulfilling career where your caregiving skills directly impact lives.
