Monitor patients' vital signs, observe changes in health status, and report findings accurately to healthcare professionals. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Administer medications following prescribed care plans while adhering to HIPAA regulations for patient privacy and confidentiality. Provide companionship and emotional support, engaging clients in social activities and ensuring their rights are respected. Assist with housekeeping tasks including cleaning, laundry, and maintaining a safe environment to promote overall well-being. Support clients in community outings or appointments using personal transportation assistance, ensuring safety at all times. Skills Proven experience working with individuals with disabilities, neurodevelopmental disorders, or in senior care environments such as assisted living or nursing homes. Strong patient observation skills to detect changes in condition or behavior promptly. Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and resident's rights. Ability to perform patient monitoring, medication administration, first aid, CPR, and basic life support procedures confidently. Familiarity with care plans development and implementation tailored to individual needs. Competence in manual handling techniques including heavy lifting and patient transfer methods like Hoyer lifts. Excellent communication skills for effective client interaction and collaboration with healthcare teams.
