Deluxe Home Care Corp. is looking for a motivated and organized Case Coordinator to join our team! We are seeking a professional, reliable, and compassionate individual with excellent communication skills who is comfortable working with clients, caregivers, healthcare professionals, and referral sources. Responsibilities Coordinate client services and follow up on referrals. Assess client needs and coordinate appropriate services. Communicate with clients, caregivers, providers, and referral sources. Generate leads and monitor referrals. Maintain accurate documentation and confidentiality. Support and supervise staff as needed. Use HHA Exchange and EVV systems. Qualifications 3+ years of Home Health Care experience required. Excellent English communication skills. Strong organizational and follow-up skills. Experience with HHA Exchange and EVV . Experience generating leads and managing referrals. Strong leadership and interpersonal skills. Ability to multitask and work independently. Professional, dependable, and team-oriented.
