Job Description

Community Support Worker (CSW) / Certified Peer Support Worker (CPSW) Join our motivated, compassionate, and client-centered team! Through housing, employment, supportive services, and behavioral health programs, we help people build stability and move toward their goals. About the Position The Community Support Worker (CSW)/Certified Peer Support Worker (CPSW) provides client-centered, community-based services that support individuals in working toward recovery, stability, and self-sufficiency. Under the supervision of the Community Support Supervisor, the
CSW/CPSW
works directly with clients and their families to identify goals, develop and maintain service plans, connect with community resources, and build skills that support successful community living. This is a highly community-based position, with at least 60% of Comprehensive Community Support Services (CCSS) provided face-to-face or in vivo, where the client is. The CSW may work with clients around housing, mental health, substance use, medical needs, employment, education, family and social relationships, legal concerns, and other identified needs. Compensation $19.00-$22.00 per hour, based on experience. Work-related mileage is reimbursed for eligible miles submitted in accordance with Agency policy. What You'll Do Create, update, and review client service plans and advance directives in accordance with agency policy. Build trusting, supportive relationships with clients and help them work toward their identified goals. Identify formal and informal support systems and connect clients with community resources. Help clients access and maintain medical, housing, vocational, financial, social, entitlement, behavioral health, and other necessary services. Help identify early signs of crisis or relapse and use crisis prevention, intervention, de-escalation, and natural support to address concerns. Support medication adherence and illness self-management as directed by the clinical team. Build interpersonal, community coping, and functional skills related to daily living, communication, housing, money management, self-care, social relationships, employment, education, and other areas as needed. Help clients identify and minimize symptoms that interfere with daily living and support relapse prevention and self-monitoring skills. Support clients in maintaining employment and school participation. Build and maintain relationships with family members, friends, neighbors, landlords, employers, educators, and other natural support. Provide follow-up to determine whether accessed services have adequately met the client's service plan goals. Transport clients in a personal vehicle to and from appointments and other necessary services. Provide at least 60% of CCSS in the community, face-to-face or in vivo, where the client is. Maintain strict confidentiality of client Protected Health Information (PHI) and substance use disorder treatment records in accordance with HIPAA and 42 CFR Part 2. Attend agency staff meetings and required training and perform other duties as assigned.
What We're Looking For Required:
At least 18 years of age. Commitment to working with people experiencing severe and persistent mental illness, substance use disorders, and co-occurring diagnoses. Strong written, verbal, and interpersonal communication skills. Ability to work with sensitivity to trauma, gender, ethnicity, culture, and other factors that affect the people we serve. Ability to assist clients with service planning, recovery/resiliency planning, crisis management, and transition planning as needed. Skills in crisis prevention and intervention and the ability to de-escalate crisis situations. Familiarity with psychosocial rehabilitation and strengths-based recovery practices. Ability to access and coordinate medical, vocational, financial, social, housing, entitlement, and other community resources. Familiarity with medical conditions, medications, symptom management, and illness self-management skills education. Ability to develop and sustain natural supports in clients' lives. Basic word processing, spreadsheet, database, and internet skills. CPR and First Aid certification. Ability to drive as an essential function of the position, including transporting clients and traveling to appointments and home visits. Requires a valid New Mexico driver's license, reliable access to a personal vehicle, and current auto insurance. The position may also require walking longer distances to residences and climbing stairs when necessary. Schedule flexibility and availability by phone in case of a client or agency emergency. Successful completion of a criminal background check and drug screen.
Preferred:
High school diploma or GED. Certification as a Peer or Family Specialist. Spanish bilingual/fluency in English and Spanish. Knowledge of case management practices. Experience working in the mental health field. Experience working with people with severe and persistent mental illness, substance use disorders, or co-occurring diagnoses.
