Duties Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using proper patient transfer methods and manual handling techniques. Monitor patients' vital signs, observe health status changes, and report findings accurately to healthcare professionals while maintaining HIPAA compliance. Prepare nutritious meals aligned with dietary restrictions and assist with feeding as needed to promote proper nutrition. Support medication administration following prescribed care plans and ensure medication safety protocols are followed diligently. Provide companionship and emotional support to enhance clients' mental well-being while respecting their resident's rights and preferences. Assist with light housekeeping tasks including cleaning, laundry, and maintaining a safe environment conducive to health and comfort. Support clients during outings or appointments by providing personal transportation assistance and ensuring safe mobility. Requirements Prior experience working with individuals with disabilities, developmental or intellectual disabilities, autism, dementia (including Alzheimer's), or in senior care settings such as assisted living or nursing homes. Knowledge of patient observation techniques, patient monitoring, and emergency response procedures including CPR and first aid certification. Familiarity with specialized equipment such as Hoyer lifts for patient transfers and understanding of care plans tailored to individual needs. Ability to perform heavy lifting safely and assist with manual handling tasks requiring physical strength. Strong communication skills for effective client interaction, documentation, and collaboration with healthcare teams while maintaining confidentiality under HIPAA regulations. Experience working in home health or hospice care environments is highly desirable. Valid driver's license and reliable transportation for client outings or appointments are preferred.
