is an industry leader in providing quality home care to the members of the communities we serve in New York City, Nassau, Suffolk, and Westchester counties since 1981. We are seeking a Full time

RN FIELD SUPERVISOR

for our Suffolk County location, based in Bay Shore. As a field supervisor, you will further Bestcare's goal for our patients to remain safely in their homes, functioning at their optimum level of independence.

Job Responsibilities:

Conduct initial home visits to assess patient needs, develop the Aide Plan of Care, and provide ongoing oversight and support Communicate with a patient's physician, insurance, and other team members to ensure delivery of appropriate patient care Supervise patient care provided by our Home Health and Personal Care Aides Participate in orientation, teaching, and evaluations of care providers Responsible for the review and maintenance of patient records

Job Qualifications:

Must be a valid Registered Nurse licensed in New York State, preferably with two (2) years of experience Must have a valid driver's license and transportation. Previous home care experience is a plus Knowledge of

EMR HHAX

a plus Bilingual English Spanish is a plus

From $75,000.00 per year
