Job Description

Home Health Aide (HHA) / Personal Care Aide (PCA) 3/12's 7pm-7am
Job Summary:
Vestal Park Rehabilitation and Nursing Center, a compassionate and dedicated healthcare organization, is seeking dedicated and caring Home Health Aides (HHAs) and Personal Care Aides (PCAs) to join our team! As a HHA/PCA, you will provide exceptional care and support to our residents, promoting their independence, dignity, and well-being. Responsibilities:
Administer medications and treatments as directed by physicians and update associated documentation Assist residents with activities of daily living, including personal care and hygiene Provide vital sign measurements and record vital signs as ordered by providers Assist residents with exercises, oxygen equipment, and ostomy care as needed Perform administrative tasks, such as completing necessary forms and reports Participate in facility surveys and inspections as required Provide personal assistance and respond to call bells or signaling calls Assist with activities, mealtime, and resident orientation Notify supervisors of any changes in resident status or medical needs Maintain confidentiality and adhere to all facility policies and procedures
Requirements:
Current and valid HHA Certificate or PCA At least 18 years of age Ability to read, understand, and follow resident care plans Strong communication and interpersonal skills Ability to work independently and as part of a team Reliable transportation and willingness to travel to resident locations (if applicable)
