This vital role involves providing personalized care and support to a kind and caring male US Veteran. Duties Assist patient with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene. Monitor patient health by observing vital signs, managing medication administration, and maintaining accurate care plans. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Maintain cleanliness of living spaces through regular cleaning duties to ensure a safe and hygienic environment. Observe and document changes in residents' conditions, communicating effectively with healthcare teams about patient status. Provide transportation assistance for appointments or community outings to promote independence and engagement. Requirements Proven experience working in client homes, assisted living facilities, nursing homes, or home health environments. Familiarity with HIPAA regulations to ensure confidentiality and privacy of patient information. Hands-on experience with dementia care, Alzheimer's care, and memory care. Ability to perform manual handling tasks safely, including heavy lifting and patient transfers using appropriate equipment like Hoyer lifts. Strong communication skills for effective client interaction and team collaboration. Flexibility to work full-time hours in various shifts as needed to provide consistent care.
