Job Summary We are seeking a compassionate and dedicated Caregiver to provide exceptional support and assistance to individuals in Medina. Responsibilities Provide supervision, assistance, and support according to each individual's ISP and assessed needs. Assist individuals with activities of daily living, including bathing, grooming, dressing, toileting, oral hygiene, and other personal-care needs. Assist with meal preparation, grocery shopping, household cleaning, laundry, and maintaining a safe and sanitary living environment. Support individuals in developing and maintaining independent living skills. Assist individuals with community activities, appointments, shopping, recreation, and social activities. Provide transportation when required and authorized by the agency. Encourage individuals to make choices and participate in decisions regarding their daily lives. Respect and protect the rights, privacy, dignity, and confidentiality of each individual. Follow behavioral support strategies and approved plans as written. Follow all health and safety protocols and immediately report concerns regarding an individual's well-being. Assist with medications only when properly trained, certified, and authorized to do so. Observe individuals for changes in behavior, health, physical condition, or emotional well-being and report concerns promptly. Complete accurate and timely service documentation, progress notes, goal/outcome documentation, and other required records. Accurately document services provided and follow applicable Electronic Visit Verification (EVV) requirements. Follow all incident-reporting requirements and immediately report unusual incidents (UI), major unusual incidents (MUI), suspected abuse, neglect, exploitation, misappropriation, or other reportable concerns according to agency policy and applicable requirements. Maintain professional boundaries with individuals, families, guardians, coworkers, and other members of the individual's support team. Communicate professionally with supervisors, coworkers, individuals, guardians, county board personnel, and other service providers. Attend required staff meetings, training sessions, and continuing education. Follow all agency policies, procedures, ISP requirements, health and safety requirements, and applicable DODD rules. Arrive for scheduled shifts on time and remain on duty until properly relieved when required. Perform other duties reasonably related to the individual's care and services as assigned. Skills Experience working with individuals with developmental disabilities, neurodevelopmental disorders, or dementia care such as Alzheimer's and memory care Strong knowledge of patient monitoring, medication administration, and first aid/CPR certification Familiarity with assisted living environments, nursing homes, home health care, hospice care, and long-term care settings Ability to perform heavy lifting safely and assist with patient transfers using appropriate equipment like Hoyer lifts Excellent communication skills for client interaction and team collaboration Knowledge of resident's rights, HIPAA compliance, and confidentiality standards Skilled in assisting with personal transportation needs and supporting clients with mobility aids such as wheelchairs or walkers Join us in providing compassionate caregiving that transforms lives!
