Job Description

As a Caregiver at Sevita, you'll provide person-centered care in home and community settings to individuals of all ages and abilities. Responsibilities include assisting with daily living activities, medication reminders, mobility support, and community outings while promoting independence, dignity, and safety. You'll collaborate with clinical teams, follow individualized care plans, and document care accurately. Responsibilities Provide hands-on assistance with activities of daily living (bathing, dressing, grooming, toileting, and hygiene) Support safe mobility, transfers, and positioning following individual care plans Offer medication reminders in accordance with company and state guidelines Assist with meal preparation, feeding, and monitoring dietary needs Accompany individuals to community activities, appointments, and outings Observe, document, and report changes in physical or behavioral condition Follow individualized service plans and collaborate with clinical and support teams Maintain a clean, safe, and respectful home environment Use de-escalation, redirection, and behavioral support strategies as trained Participate in required training, certifications, and ongoing professional development Required Skills Personal care assistance (ADLs and IADLs) Safe transfer and mobility techniques Basic vital signs monitoring and observation Medication reminder procedures Documentation and charting in healthcare settings Infection control and universal precautions Behavioral support and de-escalation techniques Home safety and fall-prevention practices Meal preparation and basic nutrition support Use of adaptive equipment and assistive devices
