We are seeking a dependable and caring Personal Care Specialist (CNA) to provide hands-on support to patients in their homes while working closely with nurses and the interdisciplinary hospice team.

Local travel throughout the Wooster service area What You'll Do As a Personal Care Specialist, you'll help patients remain comfortable and maintain their dignity and independence in the comfort of their own homes.

Your responsibilities will include:

Assist patients with activities of daily living, including bathing, dressing, grooming, toileting, and mobility Provide compassionate, respectful, and patient-focused care Observe patients for changes in condition and promptly report concerns to the appropriate member of the care team Accurately document care and services provided using electronic systems Promote patient safety, comfort, privacy, and dignity Communicate effectively with nurses, caregivers, family members, and other members of the hospice team Develop trusting relationships with patients and their loved ones Follow the individualized plan of care and hospice policies and procedures Qualifications High school diploma or GED required Current Ohio

STNA/CNA

certification preferred At least one year of healthcare or direct patient-care experience preferred Basic computer skills and ability to use electronic documentation systems Valid driver's license and safe driving record Reliable transportation and state-required automobile insurance Ability to travel locally between patient homes Ability to work in a variety of weather and driving conditions Hospice certification (CHPNA) is encouraged within two years of employment Why Join Hospice Care?
