Job Description

CAREGIVER RESPONSIBILITIES
RESIDENT
CARE & ASSISTANCE
Assist residents with ADLs, including bathing, grooming, hygiene, dressing, oral/hair care, shaving, toileting, and incontinence care. Assist with mobility, transfers, lifting, turning, repositioning, and transportation using proper equipment and techniques. Maintain resident cleanliness, comfort, dignity, and appropriate positioning; clean and sanitize toilets and surrounding areas as needed. Assist with medical appointments, examinations, tests, therapies, procedures, and end-of-life comfort measures.
MONITORING & DOCUMENTATION
Observe and promptly report changes in physical/emotional condition, injuries, skin concerns, accidents, incidents, complaints, and grievances. Immediately report suspected abuse, neglect, misappropriation, or missing residents per policy. Accurately document care, observations, incidents, vital signs, weights, intake/output, and other assigned data in PCC and required systems. Frequently monitor residents who cannot independently call for assistance.
RESIDENT ENVIRONMENT & TRANSITIONS
Greet, orient, and escort residents to rooms and community areas. Maintain clean, safe, comfortable, and respectful resident living spaces that promote dignity.
SAFETY, COMPLIANCE & INFECTION CONTROL
Follow facility policies, infection control procedures, safety standards, HIPAA, and privacy requirements. Perform only trained and authorized tasks and use equipment properly. Promptly report defective equipment, safety concerns, exposures, and unauthorized system access. Respect resident rights, preferences, and refusals of care and report concerns appropriately.
COMMUNICATION & TEAMWORK
Receive and provide shift reports and communicate effectively with nursing and interdisciplinary teams. Participate in care discussions and meetings and collaborate with all departments to support quality and continuity of care. Promptly notify supervisors of absences, tardiness, or schedule changes.
ADDITIONAL DUTIES
Assist with meal setup, feeding, hydration, and post-meal care according to service plans. Maintain clean and organized resident areas. Perform other duties within the scope of training and certification. We are an equal opportunity employer.
