Job Summary We are seeking a compassionate and dedicated Direct Care Worker to provide essential support and assistance to individuals with diverse needs. Responsibilities Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor and record vital signs such as blood pressure, pulse, and temperature to ensure health stability. Administer medications accurately following prescribed schedules and protocols. Provide specialized care for individuals with dementia or Alzheimer's disease, utilizing memory care techniques and behavior management strategies. Support residents with meal preparation, feeding assistance, and maintaining nutritional needs. Operate medical equipment such as Hoyer lifts or other assistive devices safely and effectively. Maintain a clean and safe environment by performing light housekeeping duties and adhering to infection control standards. Document care activities using electronic medical record (EMR) systems accurately and promptly. Engage residents in social activities to promote mental well-being and social interaction. Collaborate with healthcare professionals, family members, and team members to develop personalized care plans aligned with each resident's needs. Qualifications Prior experience in long-term care settings such as nursing homes, assisted living facilities, or home & community care is highly desirable. Knowledge of medication administration procedures and vital signs monitoring. Experience working with individuals with developmental disabilities or disabilities requiring specialized support. Familiarity with dementia care techniques including behavior management strategies. Certification in first aid is preferred; CPR certification is a plus. Strong interpersonal skills with the ability to communicate effectively with residents, families, and team members. Ability to lift or transfer residents using assistive devices like Hoyer lifts safely and confidently. Demonstrated caregiving skills with a compassionate approach focused on dignity and respect. Valid driver's license for driving responsibilities related to home & community care is advantageous.
