What You Will Do:

Enhance Resident Well-being Escort residents to and from activities; assist at mealtimes after completing mandatory training. Drive residents to outside activities using SPH vehicles, including 14-passenger vehicles; certify competence driving within 90 days of hire and annually. Provide companionship to minimize loneliness, helplessness, and boredom. Offer moral support when needed; confidentiality is critical.

Coordinate Activates & Care:

Schedule, organize, and conduct activities within the Home and community. Attend care plan conferences; write goals and objectives, and complete necessary documentation such as assessments, surveys, and progress notes. Develop activities and special events of interest to the residents. Complete requisitions, arrange transportation, request supplies, make needed purchases, follow through with any other actions needed for preparation and implementation. Keep accurate daily records of activities and resident involvement. Attend training, in-services, and development seminars. Maintain organized and clean activity/storage areas; assist with special events like family picnics, holiday parties, etc.; assist with setting up and cleaning up after resident/staff functions. Build meaningful relationships Identify residents' past and present interests. Address resident concerns, serve as an advocate, and work with families, volunteers, and the public on various projects.
