Job Description

Job Overview We are seeking a compassionate and dedicated Direct Care Worker to provide exceptional support and assistance to individuals with diverse needs in various care settings. In this vital role, you will be the primary caregiver, ensuring residents or clients receive personalized care that promotes independence, safety, and well-being. Responsibilities Assist residents or clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene. Monitor patient health by observing vital signs, administering medication, and reporting changes to healthcare professionals. Support patients with mobility using equipment such as Hoyer lifts and patient transfer methods while ensuring safety through manual handling techniques. Prepare nutritious meals tailored to dietary needs and assist with meal consumption to promote proper nutrition. Maintain a clean and safe environment by performing regular cleaning tasks and adhering to infection control protocols. Document care activities accurately in care plans, record vital signs, medication administration, and patient observations following HIPAA regulations. Provide transportation assistance for appointments or community outings while ensuring patient safety during transfers. Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of patient observation techniques, dementia care, Alzheimer's care, memory care, and hospice care practices. Familiarity with HIPAA regulations and maintaining patient confidentiality is essential. Certification in CPR and First Aid is required; additional certifications such as Hoyer lift training or medication administration are a plus. Experience working with individuals with autism, developmental disabilities, neurodevelopmental disorders, or intellectual disabilities support is advantageous. Ability to lift heavy objects safely and perform manual handling tasks while prioritizing safety for both yourself and the patient. Strong communication skills for effective client interaction and team collaboration in home & community care settings.
