Job Summary We are seeking a compassionate and dedicated Direct Care Worker to join our team in Doylestown, PA . In this vital role, you will provide personalized support and assistance to individuals with diverse needs, including seniors, those with developmental disabilities, and individuals living with dementia or autism. Responsibilities Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility to promote independence and dignity. Support residents with medication reminders following care plans and medical instructions, ensuring accuracy and safety. Monitor vital signs and observe behavioral changes to inform ongoing care strategies. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Maintain cleanliness of residents' living spaces through regular cleaning and sanitation practices. Document care activities accurately using Electronic Medical Record (EMR) systems and update care plans as needed. Engage residents in social activities and provide companionship to foster emotional well-being. Skills Prior experience in assisted living, nursing homes, or home care environments is highly valued. Knowledge of Alzheimer's care, memory care, dementia management, and hospice support enhances your ability to serve residents effectively. Strong understanding of medical terminology, HIPAA regulations, and patient confidentiality standards. Excellent conflict management and behavior management skills to handle challenging situations compassionately.
