Direct Care Worker (DCW)
Qualifications:
General - Staff persons who come into direct contact with consumers shall have the following prior to employment, or within 30 days of the date of hire: 1) Completed application on file. 2) Policy and Procedure signed on file. 3) Equal Opportunity on file. 4) References on file. 5) Hand washing form signed. 6) PPD completed on file. 7) Hepatitis B form completed on file. 8) Copy of Driver's License on file. 9) Copy of Social Security Card on file. 10) Copy of Car Insurance on file. 11) Copy of Criminal Background Check on file. 12) Copy of Child Abuse Clearance on file. 13) Copy of W-4 on file. 14) Copy of I-9 on file.
Education/Experience:
The personal care aide shall have at least one of the following qualifications: High school diploma GED equivalent. One or more years or related experience in working with individuals in a health Care or social service setting. Satisfactory completion of requirements of the Department of Education necessary to be a nursing assistant. Abilities - In addition to the responsibilities detailed in this job description, the Personal Care Aide must be able to: 15) understand the needs of the elderly and, in particular, the needs those with cognitive impairments (memory problems, confusion, and disorientation). 16) Be sensitive to the needs of consumer's family. 17) Be able to treat the consumer with dignity and respect. Responsibilities of the Direct Care Worker The Direct Care Worker is responsible for providing personal care as directed by the Staffing Coordinator and for assisting with bathing, dressing, grooming, meals, activities and house-keeping functions.
Responsibilities include:
1) Observation of each consumer and immediately reporting any changes in their condition to the Staffing Coordinator. 2) Under the supervision of the Staffing Coordinator, providing restorative or rehabilitative nursing services including taking consumers to the toilet at scheduled times, and assisting consumers to walk, talk and carry out their everyday activities. 3) Provide personal care services such as bathing, incontinency care, care of fingernails, etc. 4) Taking care to prevent the spread of infection. 5) Taking care to prevent breakdown of the consumer's skin. 6) Assisting with meal service and informing the Staffing Coordinator when consumers are not eating or adequately or taking fluids. 7) Assist in emergencies as directed by the Staffing Coordinator. 8) Daily straightening of the consumers living area, shower area, clothing storage area and personal care and incontinency supplies. 9) Assisting with activities when not performing the above duties.
