Overview We are seeking a dedicated Direct Care Worker (DCW) to join our team and deliver exceptional care to individuals with diverse needs. This role offers an exciting opportunity to make a meaningful difference in the lives of clients by assisting with daily activities, health monitoring, and providing compassionate support in various care settings. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and record vital signs such as blood pressure, pulse, and temperature to ensure health stability Support medication administration following prescribed guidelines while maintaining HIPAA compliance for patient privacy Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking Use specialized equipment such as Hoyer lifts or resident's to ensure safe transfers and mobility assistance Observe and document patient conditions, behaviors, and changes to inform care plans and communicate effectively with healthcare teams Maintain a clean, safe environment by performing light housekeeping tasks and adhering to infection control protocols Requirements Prior experience in group homes, assisted living facilities, nursing homes, or home & community care environments is highly preferred Knowledge of patient observation techniques, Alzheimer's care, dementia care, memory care, autism support, hospice care, and working with individuals with disabilities or developmental challenges Certification in CPR and first aid is required; additional certifications such as HIPAA training are a plus Ability to lift heavy objects safely and perform physical tasks involving mobility assistance or transfers Strong communication skills for working collaboratively with clients, families, and healthcare professionals Experience with medication administration, vital signs monitoring, and developing or following care plans is desirable Compassionate attitude with a commitment to providing respectful, person-centered care in home health or long-term care settings Join our team to help empower individuals through dedicated caregiving that enhances their quality of life.
