Overview Join our compassionate team as a Direct Care Worker and make a meaningful difference in the lives of individuals requiring personalized support and care. In this vital role, you will provide hands-on assistance with daily activities, monitor health and safety, and foster a warm, respectful environment that promotes independence and dignity. Your dedication will help enhance the quality of life for residents across various care settings, including group homes, assisted living facilities, nursing homes, and home health environments. This position offers a rewarding opportunity to engage in patient-centered care while developing valuable skills in healthcare support. Responsibilities Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity. Support patients with mobility using proper manual handling techniques and equipment like Hoyer lifts or patient transfer methods to promote safety during movement. Monitor vital signs including blood pressure, pulse, temperature, and respiration; accurately document findings in care plans while adhering to HIPAA privacy regulations. Administer medications following prescribed instructions and assist with medication administration procedures under supervision or as trained. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary, ensuring proper food safety standards are maintained. Observe patient behavior closely to identify changes or signs of health issues such as dementia or Alzheimer's disease; report concerns promptly to healthcare professionals. Maintain a clean and safe environment by performing routine cleaning tasks, assisting with laundry, and ensuring resident spaces meet health standards. Provide transportation assistance for appointments or community outings using personal transportation or agency-provided vehicles. Support residents' engagement in social activities and facilitate communication to promote emotional well-being. Follow all care plans meticulously while respecting residents' rights and preferences; participate in ongoing training related to dementia care, hospice care, neurodevelopmental disorders, or developmental disabilities when applicable. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly desirable. Demonstrated ability to perform patient monitoring, vital signs measurement, medication administration, and basic first aid including CPR certification. Familiarity with caring for individuals with dementia, Alzheimer's disease, autism spectrum disorder (ASD), intellectual disabilities or neurodevelopmental disorders enhances your effectiveness in this role. Experience supporting patients through personal transportation assistance and assisting with ADLs demonstrates versatility in caregiving skills. Knowledge of HIPAA regulations ensures confidentiality and compliance in all aspects of patient care. Hands-on experience with manual handling equipment like Hoyer lifts or patient transfer methods is preferred but not mandatory—training will be provided if needed.
