ABOUT CARE MANAGEMENT BY NATALIE
(CMN) Care Management by Natalie is a client-centered geriatric care management organization serving the SE Michigan region (Ann Arbor, Ypsilanti, Saline, Dexter, Chelsea) We take a holistic approach to supporting older adults and their families through the aging process. CMN offers comprehensive in-home assessments covering health, functional needs, home safety, nutrition, and support systems; develops care plans in partnership with clients and caregivers; connects clients to resources in the community; and provides ongoing well-being monitoring and strategies to maintain independence and quality of life. We're excited to launch a new service line in home support (direct care) to complement our existing care management services — helping clients receive hands-on assistance with daily living while maintaining our strong values of quality, compassion, and client empowerment.
POSITION SUMMARY
As a Caregiver Coordinator, you will be a pivotal figure in this new service line — combining fieldwork, caregiver leadership, and operational support.
This role is hybrid:
some hands-on care, some supervisory, and some administrative. You'll help ensure that the in-home support service meets CMN's high standards, while also building a strong, supportive team of caregivers. Flexibility is important as responsibilities will shift depending on client needs, staffing, and the growth of the service line. Over time, this role will grow in leadership scope.
KEY RESPONSIBILITIES
Field Support & Direct Care (20+ hrs/week): Provide high-quality in-home care, especially during staffing gaps or urgent client needs, to ensure seamless service and client satisfaction.
Caregiver Onboarding & Training:
Lead orientation for new caregivers; ensure compliance with regulatory, safety, and organizational care standards.
Caregiver Support & Leadership:
Act as a mentor and resource: provide coaching, feedback, and encouragement; help caregivers handle challenges and grow in their roles.
Operational & Administrative Support:
Assist with scheduling, documentation, phone triage, shift coverage, and other tasks as determined by the Director of Operations or equivalent.
Communication & Coordination:
Serve as liaison between field staff, care managers, administrative team, and clients/families to ensure clear, timely, and consistent communication.
Staffing & Coverage:
Step in as needed to cover shifts or take on extra workload to maintain continuity of care.
Reporting & Quality Assurance:
Report to the Operations Manager (or equivalent); monitor caregiver performance, client feedback, safety incidents, and schedule/staffing issues; contribute to continuous improvement.
IDEAL CANDIDATE QUALIFICATIONS 3-5
years experience in a caregiving role, home health, or similar direct care setting A genuine heart for service; compassionate, respectful, and committed to helping older adults remain safe, independent, and valued Adaptability and willingness to take on varied tasks; positive, "roll up your sleeves" attitude Strong communication skills, both verbal and written; able to coach and give feedback with empathy Patience, resilience, and ability to support caregivers through challenges and mistakes Basic tech-skills: comfort with scheduling tools, documentation systems, remote/hybrid communication Team oriented; someone who contributes to a supportive, collaborative culture
