Job Description

Home Care Program Care Manager/Caregiver Specialist Seeking a full-time (37.5hrs/wk) The Home Care Care Manager/Caregiver Specialist provides person-centered case management, care coordination, and resource navigation to older adults and family caregivers. Working within the Massachusetts Home Care Program, the position helps consumers access services and supports that promote independence, safety, and successful community living. The position also supports the Family Caregiver Support Program (FCSP) and GUIDE Model by providing caregiver support, resource coordination, respite planning, and dementia-related services. The employee serves as backup coverage for FCSP and GUIDE activities and may assume responsibility for a GUIDE caseload as program needs evolve. Qualifications and Skills Bachelor's degree preferred, with a concentration in social work, psychology, counseling, human development, nursing, sociology, criminal justice, public health, public policy, gerontology, or a related field. At least one year of experience in the human services field is preferred. A valid driver's license, reliable vehicle, and active automobile insurance are required. The successful candidate will demonstrate strong organizational, time management, prioritization, and problem-solving skills, with the ability to manage multiple responsibilities in a fast-paced environment. Must be able to maintain accurate records, pay close attention to detail, document clearly, and communicate effectively both verbally and in writing. The ability to work independently, take initiative, and collaborate effectively within an interdisciplinary team is essential. Computer proficiency, including experience with Microsoft Word and Outlook, is required. Candidates must have the physical ability to climb stairs and lift and carry items weighing up to 20 pounds.
