Multifaceted social service agency serving elderly and persons with disabilities, seeks Care Manager to assess consumer needs, conduct home visits, develop service plans and maintain case records. Bachelor's degree preferred in a related social service field. Bilingual skills and/or 2 years of experience in a related field may be substituted for education. Care management or experience with elders preferred. Must possess computer skills. Use of a reliable vehicle including active vehicle insurance with a current license to drive required.

• A staged hiring bonus will be awarded in 2 parts, half at 120 Days, and half at 1 year. The bonus for this position is $2,500.
