Duties Deliver comprehensive personal care assistance, including help with bathing, grooming, dressing, and mobility to promote comfort and dignity. Assist clients with Activities of Daily Living (ADLs) such as toileting, feeding, and transferring using proper patient transfer methods like Hoyer lifts or manual handling techniques. Monitor patients' vital signs and observe for any changes in health or behavior, documenting findings accurately while adhering to HIPAA privacy regulations. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation to support overall health. Support clients with medication administration following care plans and ensuring safety during medication routines. Provide transportation assistance for appointments, errands, or community activities to foster social engagement and independence. Maintain a safe environment through light cleaning duties, sanitation practices, and ensuring equipment such as Hoyer lifts are properly maintained. Collaborate with healthcare professionals on care plans, patient monitoring, and updates to ensure consistent quality of care. Offer specialized support for residents with Alzheimer's disease, dementia, autism spectrum disorder, or other neurodevelopmental conditions through patience and tailored approaches. Respond promptly to emergencies by applying first aid or CPR when necessary and coordinating with emergency services if required. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to observe patients closely and report changes accurately is essential. Experience working with individuals with disabilities—including developmental or intellectual disabilities—is a strong asset. Knowledge of home & community care practices including hospice care, geriatrics, memory care, and long-term care is beneficial. Familiarity with patient transfer methods such as Hoyer lifts or other manual handling techniques ensures safe mobility support. Certification in CPR and first aid is required; additional certifications related to healthcare or caregiving are advantageous. Ability to work independently while maintaining excellent client communication skills is vital for success in this role.
