Job Summary The Independent Living Coordinator plays a vital role in ensuring the well-being and quality of life for individuals looking to improve their independence. This position involves overseeing weekly coordinating of care services, and providing support to individuals with developmental disabilities, autism, and behavioral health needs. Duties Coordinate and oversee weekly scheduling with individuals to ensure a safe and supportive environment. Oversee individualized care plans in collaboration with the consumer, families, and healthcare professionals. Provide in person support to individuals, assisting with daily living activities and promoting independence. Implement behavior management strategies to address individual needs and enhance overall well-being. Communicate effectively with consumers, families, and staff to foster a positive community atmosphere. Willing to learn and train on best practices for supporting individuals with disabilities, including those on the autism spectrum. Maintain accurate documentation of consumer's progress and care plans while ensuring compliance with regulatory standards. Facilitate activities that promote social interaction, skill development, and community engagement. Requirements Bachelor's degree in Social Work or related field preferred; relevant experience may substitute for education. Experience working with individuals with developmental disabilities, autism, or behavioral health challenges is highly desirable. Proficiency in Spanish speaking, writing, and reading is preferred. Strong communication skills, both verbal and written, to effectively interact with diverse populations. Ability to work collaboratively within a team-oriented environment while also demonstrating independent problem-solving skills.
