Home Health Caregiver Job Summary:

Wellhaven Home Care Charleston is seeking a compassionate and dedicated Home Health Caregiver to join our team of healthcare professionals in Myrtle Beach, South Carolina. As a Home Health Caregiver, you will have the opportunity to make a meaningful difference in the lives of our patients and their families by providing high-quality, personalized care and support.

Responsibilities:

Provide personal care and assistance with daily living activities, such as bathing, dressing, and feeding, to patients in their homes Assist with medication management, including administering medications as prescribed by physicians Monitor and report changes in patients' conditions to healthcare providers Maintain accurate and detailed records of patient care and services provided Collaborate with healthcare teams to develop and implement individualized care plans Provide emotional support and companionship to patients and their families Perform light housekeeping and laundry tasks, as needed

Requirements:

Home Health Caregiver certification or equivalent experience Ability to lift and transfer patients, as needed Excellent communication and interpersonal skills Ability to work independently and as part of a team Reliable transportation and valid driver's license (if applicable)
