Responsibilities Assist residents with Activities of Daily Living (ADLs), including personal hygiene, dressing, grooming, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods. Monitor residents' health by observing patient conditions, and reporting changes promptly to healthcare teams or supervisors. Prepare nutritious meals, assist with meal consumption, and support dietary needs aligned with individual care plans. Support residents during community outings or appointments by providing personal transportation assistance and ensuring safety at all times. Maintain a clean and safe environment through regular cleaning duties, laundry services, and adherence to infection control protocols. Document care activities accurately in resident records, including patient monitoring notes, medication administration logs, and incident reports. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of dementia care, Alzheimer's care, memory care, autism support, intellectual disabilities support, and neurodevelopmental disorders is advantageous. Ability to observe patients closely for changes in condition or behavior while maintaining patient rights and dignity at all times. Strong communication skills for effective interaction with residents, families, team members, and healthcare professionals. Physical ability to perform heavy lifting, assist with patient transfers using proper techniques, and stand or walk for extended periods. Familiarity with HIPAA regulations ensures confidentiality of all resident information.
