Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using proper patient transfer methods such as Hoyer lifts and manual handling techniques. Monitor patients' vital signs and observe for any changes in health status, promptly reporting findings to healthcare professionals. Administer medications accurately following care plans and ensure compliance with HIPAA regulations to protect patient privacy. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Support clients with cognitive impairments such as dementia or Alzheimer's disease through specialized memory care techniques and patient observation. Provide transportation assistance for appointments, errands, or community activities to promote social engagement and independence. Maintain detailed documentation of patient care activities, medication administration, and progress notes while respecting residents' rights and confidentiality. Qualifications Prior experience working with individuals with disabilities, developmental or intellectual disabilities support, autism spectrum disorder, or neurodevelopmental disorders is highly desirable. Demonstrated experience in group home settings, nursing homes, assisted living facilities, or home health environments. Knowledge of patient monitoring techniques, first aid procedures, CPR certification, and safe manual handling practices. Ability to effectively communicate with clients, families, and healthcare teams while demonstrating compassion and patience. Familiarity with care plans, medication administration protocols, HIPAA compliance, and resident's rights is essential. Strong organizational skills to manage multiple responsibilities efficiently in a fast-paced setting. Physical ability to perform heavy lifting when necessary and assist with patient transfers safely.
