Duties Provide personal care assistance including bathing, grooming, dressing, and mobility support to clients with diverse needs Assist with activities of daily living (ADLs) such as meal preparation, light housekeeping, and laundry to maintain a clean and safe living environment Monitor patients' health by observing vital signs, medication administration, and overall well-being, ensuring adherence to care plans Support clients with mobility using proper manual handling techniques and equipment like Hoyer lifts or patient transfer methods Help clients with transportation for appointments, errands, or social activities to promote community engagement Document care activities accurately while respecting residents' rights and confidentiality in compliance with HIPAA regulations Respond promptly to emergencies with first aid or CPR when necessary, ensuring safety at all times Skills Proven experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care environments Strong knowledge of patient observation, dementia care (including Alzheimer's and memory care), and neurodevelopmental disorders Skilled in assisting with ADLs, medication administration, patient monitoring, and vital signs measurement Familiarity with home health practices, hospice care procedures, and HIPAA compliance standards Ability to operate patient transfer equipment such as Hoyer lifts safely and effectively Excellent communication skills for client interaction and team collaboration Physical stamina for heavy lifting, manual handling, and assisting with patient transfers; capable of supporting clients during long shifts Join us to provide compassionate home & community care that truly makes a difference!
