In this vital role, you will deliver high-quality assistance in clients' homes, promoting independence, safety, and well-being. Whether assisting with daily activities or providing specialized care, your efforts will directly enhance the quality of life for those in your care. Duties Provide personal care assistance including bathing, grooming, dressing, and mobility support to clients with diverse needs. Assist with activities of daily living (ADLs) such as meal preparation, light housekeeping, laundry, and transportation to appointments or community activities. Monitor patients' health by observing vital signs, managing medication administration, and reporting any changes to healthcare professionals. Support clients with memory care needs, including dementia or Alzheimer's care, ensuring a safe and calming environment. Use proper patient transfer methods and equipment such as Hoyer lifts to ensure safe manual handling and prevent injury. Maintain detailed care plans tailored to each client's unique requirements while adhering to HIPAA regulations to protect privacy. Communicate effectively with clients and their families to ensure comfort, understanding, and satisfaction with the care provided. Skills Proven experience working with individuals with disabilities, developmental or intellectual disabilities, or neurodevelopmental disorders. Strong knowledge of patient observation techniques, vital signs monitoring, and medication management. Hands-on experience in assisted living facilities, nursing homes, or home health environments is highly desirable. Ability to perform light housekeeping tasks such as cleaning and meal preparation efficiently. Familiarity with patient transfer methods including Hoyer lifts and patient personal care assistance techniques. Knowledge of dementia care principles, Alzheimer's care strategies, and memory support best practices. Certification in CPR and First Aid; experience working within HIPAA guidelines is essential for maintaining confidentiality. Excellent communication skills for engaging with clients respectfully while supporting their resident's rights and independence. Physical stamina for heavy lifting, manual handling, and assisting with mobility across various settings.
