Responsibilities Provide personal care assistance including bathing, dressing, grooming, and mobility support to clients with diverse needs. Assist with activities of daily living (ADLs) such as eating, toileting, and medication reminders in accordance with care plans. Monitor patients' health by observing vital signs, reporting changes, and maintaining accurate documentation. Support clients with mobility using equipment like Hoyer lifts or patient transfer methods to ensure safe movement. Prepare nutritious meals and assist with meal planning to meet individual dietary requirements. Maintain a clean and safe environment through regular cleaning duties and adherence to health and safety protocols. Offer companionship and emotional support, fostering a respectful and engaging atmosphere for clients with dementia, Alzheimer's, or other cognitive impairments. Assist with transportation for appointments or community outings, ensuring safety during travel. Follow HIPAA guidelines to protect client confidentiality and uphold residents' rights at all times. Collaborate with healthcare teams to implement care plans tailored to each individual's needs. Requirements Previous experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and neurodevelopmental disorders is advantageous. Ability to perform manual handling tasks including heavy lifting and patient transfers safely. Certification in CPR and First Aid is required; additional training in medication administration or hospice care is a plus. Familiarity with patient observation techniques, vital signs monitoring, HIPAA compliance, and resident's rights is essential. Strong communication skills for effective client interaction and teamwork. Experience working with individuals with disabilities or developmental disabilities supports success in this role. Ability to handle physically demanding tasks while maintaining compassion and professionalism. Valid driver's license or reliable transportation may be necessary for client outings or home visits.
